2022 Certificate of Need Annual Survey due April 28, 2023

The annual survey from Certificate of Need (CON), which is required to be completed by all nursing facilities in the state, are now available and are due April 28, 2023. The Michigan Department of Health and Human Services provides instructions to complete the survey.

For missing facilities, licensed beds/bed day discrepancies, data criteria questions, and questions about survey fees, please email Cliffaney Wilkinson or Amanda Curtis, MDHHS Certificate of Need Evaluation Section. Please provide a valid contact number within the email where you can be reached in case a call is necessary.
