Nominate Someone Today! HCAM Awards of Excellence

Nominations are open for the 2024 Health Care Association of Michigan Long-Term Care Awards of Excellence to honor individuals and teams in the skilled nursing and assisted living sectors across Michigan. We encourage HCAM members to submit nominations.

Start the process today. Nominations close July 8, and it takes time to gather all the needed information and materials. Winners will be recognized at the HCAM Annual Convention, September 16-18, 2024, in Grand Rapids.

All HCAM members are eligible for the following awards:

• Gerry Baker Hero of Health Care Award. In 2024, HCAM will award Gerry Baker Hero of Health Care Awards to a maximum of 4 SNF employees and 4 assisted living employees.
• Team of Excellence Award. One team from a skilled nursing facility and one team from an assisted living community will be selected to receive this award.
• Resident of the Year Award. This award will celebrate one skilled nursing facility resident and one assisted living community resident who makes a significant contribution to the well-being of fellow residents, the facility and/or the long-term care community.

Skilled Nursing Facility members are eligible for the following award:

• David Lebenbom Memorial Scholarship Award. Scholarship applicants must be employed by an HCAM skilled nursing facility member in good standing for a minimum of one year and be enrolled in an education program related to LTC at an accredited college/university. This can include, but is not limited to, administration, nursing, occupational therapy and other related fields. In 2024, HCAM will award up to eight $2,500 scholarships. Get the details here.
