HCAM Terms and Conditions

Membership Dues
♦ Membership dues are based on the number of beds in operation in a skilled nursing facility. Beds in operation
are licensed beds minus beds in a Medicaid non-available bed plan. You must notify HCAM of a NABP for a dues
adjustment.
♦ Assisted living dues are based on the number of units/rooms in communities.
♦ Dues consist of state and national affiliation fees and special assessments.
♦ Dues will be billed quarterly and are non-refundable. Payment options include check and ACH.
♦ Dues must be paid within 30 days from receipt of invoice.
Membership Cancellation
♦ To cancel membership – operators must send written notice to HCAM.
♦ Membership in HCAM may be suspended when accounts become delinquent – until payment arrangements have
been made. Suspended memberships are not eligible for the member rate for educational programs
and conferences.
♦ HCAM reserves the right to terminate any membership that is in violation of these terms and conditions.
Membership of Multi- Facility Organizations
♦ Multi-facility organizations include any combination of members with one or more facilities, owned or
managed by the same corporation, limited liability company, single partnership, single proprietor, or any
other legal entity.
♦ All facilities owned or managed within the state of Michigan and having a majority interest or control by the
same legal entity shall be held in membership.
♦ AHCA/NCAL dues are based on the individual facility, capped at 200 beds/units per building, and do not
qualify for the discount.
Skilled Nursing Facilities Dues Structure
♦ Multi-facility organizations with more than 2,000 beds in Michigan qualify for a corporate discount on a
single master invoice each quarter.
♦ Multi-facility organizations will receive a 50 percent discount on all beds in excess of 2,000 for HCAM
dues and special assessments.
Assisted Living Communities Dues Structure
♦ Multi-facility organizations with more than 750 units in Michigan qualify for a corporate discount on a
single master invoice each quarter.
♦ Multi-facility organizations will receive a 100 percent discount on all units in excess of 750 for HCAM
dues and special assessments.
Membership Guidelines
♦ Members are prohibited from sharing members-only communications or resources with non-members. All
materials are proprietary and protected.
♦ Members are required to update HCAM of staffing changes to limit disruptions in receiving member
communications.
